Sumario:Intestinal transplantation (ITx) is a reliable therapeutic option to treat patients that suffer irreversible chronic intestinal failure with complications associated to parenteral nutrition.\nDespite of improvement in ITx due to the application of new immunosuppression protocols, acute cellular rejection (ACR) has a higher incidence than in other organs and still be cause of, both early and delayed, graft loss and needs of re-transplantation.\nIn this work, cellular targets of apoptosis in the intestinal crypt during ACR were studied by immunohistology techniques on biopsies from ITx patients. Intestinal stem cells and Paneth cells remained intact while the differentiation intermediaries of epithelial cells zone was seriously damaged.\nAlso, the roll of innate lymphoid cells subgroup 3 (ILC3s) as intestinal homeostasis regulators was evaluated using flow cytometry. This population was not affected by immunosuppression and, in ACR samples, showed a pro-inflammatory phenotype. Consistently IL-22 expression, an essential cytokine in epithelial regeneration, decreased.\nIncomplete epithelial repair during ACR, in presence of intact stem cells, may be due to the detriment in IL-22. This open a new therapeutic option different to immunosuppressant drugs.
