The definitive typification it demands from 24 to 48 hours. The technique of fluorescent in situ hybridization (FISH), is a rapid method for the identification of bacteria in blood culture. Aims: The aims of this work was to establish the FISH technique directed to the 16S RNA of SAU in samples of blood cultures with CGPR to the Gram staining, to determine the concordance with the method of reference, and to evaluate the possibility to use as standard method in the laboratory of microbiology. Material and Methods: The blood cultures analyzed in the Laboratory of Microbiology of the Hospital Privado Cordoba from 01.01.2009 to 31.12.09 were included in this study. To each positive blood culture which showed CGPR was applied simultaneously biochemical identification and FISH. Results: From 496 positive samples, 32 showed CGPR, 24 of them were identified as SAU. A concordance of 100% (IC95% 99 to 100) between both methods of diagnosis, with a value of k<0.01 was observed. Discussion: The excellent concordance opens the prospect for a study in order to determine the predictive value of FISH, its sensibility and specificity. Furthermore, it will be possible to evaluate its clinical impact being incorporated in a laboratory of clinical bacteriology. |

Antecedentes: La técnica de referencia para la detección de bacteriemias es el hemocultivo. Uno de los gérmenes que causa bacteriemias con mayor frecuencia es Staphylococcus aureus (SAU). La presencia de cocos Gram positivos en racimos (CGPR) en un hemocultivo es sugestiva de bacteriemia, aunque la relevancia del hallazgo depende de la identificación correcta del microorganismo. La tipificación definitiva demanda entre 24 y 48 hs. La técnica de hibridación fluorescente in situ (FISH), es un método rápido para la identificación de bacterias en hemocultivos. Objetivos: El objetivo de este trabajo fue poner a punto la técnica de FISH dirigido al ARN 16s de SAU, en muestras de hemocultivos con CGPR a la coloración de Gram, determinar la concordancia con el método de referencia y la factibilidad de implementarlo en la rutina del laboratorio de Microbiología. Material y métodos: Se incluyeron los hemocultivos estudiados en el Laboratorio de Microbiología del Hospital Privado de Córdoba, entre el 1/01/09 y 31/12/09. A cada hemocultivo positivo que mostró CGPR se les realizó simultáneamente identificación microbiológica bioquímica y FISH. Resultados: Sobre 496 positivas 32 mostraron CGPR y 24 se identificaron como SAU. Se observó una concordancia de 100% (IC95% 99 a 100) entre ambos métodos diagnósticos, con un valor de ? <0,01. Discusión: Las excelente concordancia obtenida abre la puerta a un estudio para determinar el valor predictivo de FISH, su sensibilidad y especificidad; y evaluar el impacto clínico de su incorporación a un laboratorio de bacteriología clínica.
