Clinical trials carried out in France by Porter, Varendi and Marlier, and in Italy by Bartocci showed that olfactory stimuli to two centimeters from the nose of neonates generate appetitive or aversive physiological responses in heart rate and respiratory and facial behavior; but there are not studies that evaluate these responses at other distances. The objective of this work was to comparatively analyze the physiological (heart rate, transcutaneous and cerebral oxygen saturation) and facial behavioral responses in hospitalized newborns to two modes of olfactory stimulation, and to investigate whether the smell of mother's milk is perceive at 15 cm in the same way as at 2 cm. We realized non-blind, cross-sectional, experimental non-pharmacological clinical trial, approved by the CIEIS of the HUMN; project SECYT Formar 2018-2019. Twenty-two term neonates with weight according to their gestational age, 11 men and 11 women, were randomized into two groups: one for olfactory stimulation at 2cm (n 11) and another at 15 cm (n 11). Sensors were placed in the frontal region for brain monitoring of oxygen by near infrared spectroscopy (NIRS), and pulse oxygen saturation in the right hand. Basal, stimulation and post-basal state were also evaluated. All procedures were filmed for analysis. The data obtained were subjected to ANOVA for repeated measures, median and ranges. In both groups we observed appetitive facial responses to the presence of the smell of breast milk; there were no changes in heart rate and transcutaneous oxygen saturation when comparing the three moments of the trials; cerebral oxygen saturation showed a decrease in the values ​​at the moments of stimulation of both the right and left hemispheres, attributable to an increase in activity neuronal. We conclude that neonates detect the smell of breast milk both at 15 cm and 2 cm, which would allow creating an aerolization system in the incubators at a distance away from the nose, to stimulate the attachment and development of hospitalized newborns

The data obtained were subjected to ANOVA for repeated measures, median and ranges. In both groups we observed appetitive facial responses to the presence of the smell of breast milk; there were no changes in heart rate and transcutaneous oxygen saturation when comparing the three moments of the trials; cerebral oxygen saturation showed a decrease in the values ​​at the moments of stimulation of both the right and left hemispheres, attributable to an increase in activity neuronal. We conclude that neonates detect the smell of breast milk both at 15 cm and 2 cm, which would allow creating an aerolization system in the incubators at a distance away from the nose, to stimulate the attachment and development of hospitalized newborns Resumen:  El olfato es  imprescindible en el periodo perinatal para la supervivencia  de los  mamíferos, acceso a fuente alimenticia y nexo psico-biológico madre-hijo.Ensayos clínicos  realizados en Francia por Porter,Varendi yMarlier ,y en Italia por Bartocci demostraron que los estímulos olfativos a dos centímetros de la nariz de los neonatos generan respuestas fisiológicas en la frecuencia cardiaca y respiratorias  y comportamentales faciales apetitivas o adversivas ; no encontrándose estudios que evalúen estas respuestas  otras distancias.  El objetivo de este trabajo fue analizar comparativamente las respuestas  fisiológicas (frecuencia cardiaca, saturación transcutanea y cerebral de oxígeno) y comportamentales faciales, en recién nacidos a términos hospitalizados frente a dos modos de estimulación olfativa, e investigar si el olor a leche de madre se percibe a 15 cm  de la misma forma que  a 2 cm. Ensayo clínico, no ciego, transversal, experimental no farmacológico aprobado por el CIEIS del HUMN.Proyecto SECYT Formar 2018-2019.  Se estudiaron 22 neonatos a término con peso acorde a su edad gestacional ,11 varones y 11 mujeres,  aleatorizados  en dos grupos: uno para estimulación olfativa  a 2cm(n 11)  y otro a  15 cm(n 11); se les colocó sensores en la región frontal  para el monitoreo cerebral de oxígeno por espectroscopia infraroja cercana(NIRS), y  de saturometría de oxígeno de pulso en mano derecha.Se evaluó  el estado basal,el de estimulación y el  post basal .Los procedimientos fueron filmados para su  análisis. Los datos obtenidos fueron sometidos a ANOVA para medidas repetidas, mediana y rangos.  En ambos grupos observamos respuestas faciales apetitivas  ante la presencia del  olor a leche materna; no hubo cambios en la frecuencia cardiaca y saturación transcutanea de oxígeno comparando los tres momentos de los ensayos;la  saturación de oxígeno cerebral presentó descenso de los valores en los momentos de estimulación tanto del hemisferio  derecho como del izquierdo, atribuible a un aumento de la actividad neuronal. Concluimos que los neonatos  detectan  el olor a leche materna tanto a  15 cm como a 2cm, lo que  permitiría crear un sistema de aerolización  en las incubadoras  a una distancia alejada de la nariz ,para estimular el apego y desarrollo del recién nacidos hospitalizados. .
