The objective of this research was to identify and characterize socio-health scenarios (SHS) in America from March 2020 to June 2021. An ecological, transversal and descriptive study was conducted and data from different international organizations was used (World Health Organization, Panamerican Health Organization, United Nation´s Development Program, Global Cancer Observatory and Our World in Data). Information regarding the following was taken into account: prevalence of obesity, hypertension and diabetes, incidence of cancer, % of older adult population, total population, Human Development Index (HDI), Coverage of essential services, Health spending per capita, Health spending as% of Gross Domestic Product (GDP), COVID-19 cumulative incidence (CI), cumulative mortality (CM) and lethality (L) and government rigor index (IRG) of 27 American countries. Principal component factor analysis was applied to construct SSS, considering an eigenvalue> 1 and the Kaiser-Meyer-Olkin (KMO) sample adequacy test> 0.50 (Stata-v14 software). The identification of each SSS (factor) was based on the factor loading criterion (LC) ≥ 0.50. Four different scenarios were identified in America that explains 80% of the variability and KMO=0.64. The first scenario was called “of developed countries” and was characterized by high LC for obesity, cancer, % of older adults, HDI, health spendings per capita and as% of GDP and CI of COVID-19. Countries that presented the highest adherence to this scenario were the United States and Canada. Honduras and Haiti showed the least adherence. The second scenario named “worst affected by COVID-19” presented elevated LC for coverage of essential services, CM y L by COVID-19. Peru and México showed the most adherence and Cuba and Cánada the least. The third scenario was named “unrelated to COVID-19” and presented low LC for hypertension and diabetes.  The fourth scenario, “of high incidence in rigorous countries” presented elevated LC for governmental rigor and CI for COVID-19, being Argentina and Chile among the countries that showed most adherence to this scenario. In America 4 different scenarios were identified for COVID-19, related to development conditions, health and policies adopted by governments.  These would allow directing health promotion and preventing actions.

Resumen:  La enfermedad por SARS-CoV-2 se ha convertido en una crisis de salud mundial. Esta y las enfermedades no transmisibles (ENT) provocaron un mayor riesgo de complicaciones y muerte. El objetivo de la presente investigación fue identificar escenarios socio-sanitarios (ESS) en América desde marzo 2020-junio 2021, para conocer sus características y direccionar su abordaje. Estudio ecológico, transversal, descriptivo que utilizó datos de la Organización Mundial de la Salud, Organización Panamericana de la Salud, Programa de Naciones Unidas para el Desarrollo, Observatorio Global del Cáncer y Our World in Data. Se consideró: prevalencia de Obesidad, Hipertensión, Diabetes, Incidencia de Cáncer, % de adulto mayor, Población, Índice de Desarrollo Humano (IDH), Cobertura de servicios esenciales, Gasto en salud per cápita, Gasto en salud como % del Producto Bruto Interno (PBI), tasa de incidencia (TIA), mortalidad (TMA) y letalidad acumulada (LA) por COVID-19 e índice de rigurosidad gubernamental (IRG) de 27 países americanos. Se aplicó análisis factorial de componentes principales para construir ESS, considerando un autovalor>1 y el test de adecuación muestral Kaiser-Meyer-Olkin (KMO)>0,50 (software Stata-v14). La identificación de cada ESS (factor) se basó según el criterio de carga factorial (CF) ≥ 0,50.  Se identificaron 4 ESS en América, explicando el 80% de la variabilidad y KMO=0.64. El primero se denominó “de países desarrollados” caracterizado por altas CF para obesidad, cáncer, %adultos mayores, IDH, gastos en salud y TIA de COVID-19. Los países de mayor adhesión fueron Estados Unidos y Canadá y los de menor, Honduras y Haití. El segundo denominado “de peor impacto por COVID-19”, presentó elevadas CF para cobertura de salud, TMA y LA por COVID-19 (Perú y México-mayor adhesión- y menor adhesión Cuba y Canadá. El tercero: “No relacionado con COVID-19” presentó baja CF para Hipertensión y Diabetes. Y el cuarto ESS “de elevados contagios en países rigurosos” tuvo CF elevada para rigurosidad gubernamental y TIA por COVID-19, encontrándose Argentina y Chile entre los países que más adhirieron. En América 4 diferentes ESS se identificaron para COVID-19, relacionados a condiciones de desarrollo, salud y políticas adoptadas, que permitiría direccionar acciones de promoción y prevención de la salud. .
