Historical institutionalism is one of those approaches, which through the identification of an event which generates a subsequent way allows understanding how institutions develop and of the consequences, whether institutional or not. Law 18610 from the year 1970 which regulates the functioning and funding of health insurance impacts as the transcendental landmark in the making up of the Argentinian health system, usually described as fragmented and of poor performance. This law determined the subsequent way regarding the manner to secure citizens and in general terms the ground rules affecting the involved institutional actors (the state, the health insurance sector, and the private sector). The objective of this work is to analyze the fragmentation in health of the Argentinian health system from an alternative point of view, in order to get a better understanding of the system and offer breakthrough ideas for the system. |

Los sistemas de salud son esencialmente sistemas sociales complejos que requieren la observación de fenómenos trascendentes a través de múltiples enfoques para su correcta comprensión. El institucionalismo histórico es uno de esos enfoques, que a través de la identificación de un suceso que genera un camino ulterior permite comprender como se desarrollan las instituciones y sus consecuencias, intencionales o no. La ley 18.610 del año 1970 que regula el funcionamiento y financiamiento de las obras sociales impresiona ser ese hito trascendental en la configuración del sistema de salud argentino, descripto comúnmente como fragmentado y de bajo desempeño. Esta ley determinó el camino ulterior en cuanto a el modo de asegurar a los ciudadanos y en términos generales las leyes del juego de los actores institucionales involucrados (el estado, el sector de obras sociales y el sector privado). Este trabajo se propone analizar desde una mirada alternativa el fenómeno de fragmentación en salud del sistema de salud argentino con el objeto de lograr una mejor comprensión de este y aportar al debate de un sistema superador. Os sistemas de saúde são essencialmente sistemas sociais complexos que requerem a observação de fenômenos marcantes por meio de múltiplas abordagens para sua correta compreensão. O institucionalismo histórico é uma dessas abordagens, que através da identificação de um evento que gera um camino ulterior nos permite entender como as instituições se desenvolvem e suas consequências, intencionais ou não. A Lei 18.610 do ano de 1970 que regulamenta o funcionamento e o financiamento das obras sociais parece ser esse marco transcendental na configuração do sistema de saúde argentino, comumente descrito como fragmentado e ineficiente. Esta lei determinou o caminho a seguir em termos de como assegurar os cidadãos e em termos gerais as leis do jogo dos atores institucionais envolvidos (o Estado, o setor da segurança social dos sindicatos e o setor privado). Este trabalho pretende analisar a partir de uma perspectiva alternativa o fenômeno da fragmentação da saúde do sistema de saúde argentino, a fim de melhor compreendê-lo e contribuir para o debate sobre um sistema que possa superá-lo.
