En ciertas ocasiones los grandes escritores son además excepcionales clínicos o sintomatólogos que llegan a subvertir, anticipar y renovar los discursos sobre la subjetividad. Sintomatólogos del deseo supeditado al contrato es, en primer lugar, Leopold Sacher-Masoch cuando muestra detalladamente, en La Venus de las pieles (1870), la dinámica de lo que a partir de su nombre se llamará masoquismo, es decir, el goce de posponer la satisfacción del deseo, valiéndose para ello del suspenso, el balbuceo y la espera. Y en segundo lugar, Marguerite Duras cuando dilucida El mal de la muerte (1982), en el cual se goza en la correspondencia del deseo y la imposibilidad, correlación que da lugar a un decir fluido y neutro, reverberante. Cada una de estas elaboraciones literarias indica que la sintomatología se halla “casi fuera de la medicina, en un punto neutro, un punto cero, donde artistas, filósofos, médicos y enfermos pueden encontrarse” (Deleuze).
